Hallstrom Homeschool Workshops/F.I.R.E. Sports Application

Whether your family is interested in taking classes at Hallstrom or solely participating in our F.I.R.E. athletic programs, all families must begin the process by completing an application. At this time, the space we have available is for incoming 8-12 graders or students who are interested in F.I.R.E. sports. For those of you who fall into either of these groups, please feel free to complete our online application. We keep this page updated with the latest information on our availability. Please check back often.

It's important to note that Hallstrom does not guarantee that all applicants will be accepted or provided an opportunity to interview, as we will carefully review each application to ensure that we will be a good fit for your family and that we will have the opportunities available that you are looking for. Should your application be considered, our new family director will be in touch to arrange a time to meet for an interview.

1. If you were not a member of Hallstrom in the past, were you involved in another homeschool support group or co-op? If yes, what teaching or other responsibilities did you fulfill in that group? 2. Are both parents Christians? Briefly share your Christian walk(s) or testimony. 3. Where do you attend church? 4. Briefly describe your and your spouse's approach to discipline with your children. 5. If you or your child does not agree with an item in the Hallstrom Code of Conduct or a Rockford F.I.R.E. sports policy (if applicable), how would you address that with your child? 6. How important are academics in your family's approach to home schooling? Please explain. 7. Have your older children (grades 7-12) come to faith in Christ? How well do they receive instruction, and how do they respond to authority and discipline? 8. Have you removed your child(ren) from a traditional school setting to be home schooled? If so, when, and for what reason(s)? 9. Have any of your children ever been, or are they currently, suspended or expelled from a public or private school? If so, please explain. 10. Have any of your children ever been disciplined by school authorities for or been involved in the use of drugs, alcohol, firearms, or weapons at school? If yes, please explain. 11. Are any of your children currently taking medication for or been diagnosed with ADD, ADHD, or any other physical, behavioral, or mental disability or disorder? If yes, please explain. This information is released only to the Hallstrom Board and any of your students' teachers in order to serve you better. You need to understand, though, that we are not formally trained to deal with children who have special-needs. 12. Who will be the primary teacher at home? Does the primary teacher also work outside of the home? What is the involvement of the other parent in the student's education? What do they do for a living? 13. If your child(ren) is/are participating in Rockford F.I.R.E. sports, is he/she homeschooled for at least 51% of their current education? If you are not planning to participate in Rockford FIRE sports, you may answer this question N/A. 14. Do you understand that Hallstrom does NOT give credit toward high school graduation, although parents may use Hallstrom classes as credits toward fulfilling some graduation requirements in a given subject, according to the parents' own discretion? 18. HHSW INTEGRITY COVENANT FOR 8TH-12TH GRADERS

I do hereby give my word that I will come to class with my heart and mind prepared to learn. I will enter each class with a teachable spirit. I will be attentive and respectful of both my teachers and my fellow classmates. I agree to conduct myself at all times in a manner that glorifies God and brings honor to His name.

I agree that when I fall short of keeping this commitment, I will receive correction with humility, turn from my error, and prayerfully seek the Lord’s forgiveness and help to regain my commitment.

I understand and agree that I am responsible for completing my assignments on time and for doing them to the best of my abilities because that is my “good and faithful service” to the Lord.

Each 8-12 grade student should type their full name below to indicate they have read and agree that they abide by the rules listed in the CODE OF CONDUCT. Hallstrom students are expected to become familiar with and abide by the standards outlined in this policy to reflect Biblical standards, values, and character.
